In this case, the presentation will provide practical learning tools on the issues affecting care coordination in homeless shelters as a community organization. I believe that every person will be able to gain vital knowledge concerning the presentation.

Ethics in Healthcare

Ethics refers to a collection of moral principles that have been used within our modern-day society to manage the behavioral practice of every person. Overtly, ethics has been applied within the healthcare system to govern how medical nurses behave. It strictly contributes to most nurses choosing right over wrong when conducting health care services to every patient. This implies that ethical practices within healthcare clinics result in ineffectual decision-making from the nursing perspective. This is because nurses can provide decisive choices that are interrelated with the upright standards of society. Consequently, they result in healthcare development and growth in different communities. It is imperative to introduce ethical conduct in the provision of care coordination when focusing on homeless shelters, which is a community support group that assists homeless people.

Healthcare Policies

The healthcare policy refers to the actions that have been anticipated and implemented by the health organization in governing specific people who, in this context, are the medical personnel. The policies are established to achieve a lucid outcome. The government has been given the mandate to create policies that enhances the health efficiency and effectiveness of netizens’ life within the country. Health policies have been developed within our contemporary society to improve quality healthcare delivery to community members and reduce high levels of discrimination and other social factors. This implies that policy factors implemented within the healthcare system positively affect health outcomes. The policies can be ratified within the community organization of homeless shelters to improve the services rendered in the community.

Care Coordination

Care coordination refers to organizing and planning effective patient care among different interdisciplinary staff members when sharing information with the participant involved. Implementing policies and ethical factors should be fundamentally applied to enhance care coordination (Steaban, 2016). Precisely, care coordination in organizing home shelter programs will strengthen the healthcare needs of every member, thus reducing health disparities significantly. In other words, care coordination ties people while ensuring active participation in home shelter programs within the community to receive quality health care services. Medicare and Medicaid health care policy showcase ethical dilemmas due to the diversification of the current society. This makes it difficult for the health organization to identify individuals covered by this health program.

Impact of Ethical Principles

Ethical factors in care coordination should be administered when conducting healthcare services. It involves individuals doing right over wrong within the healthcare system. If used appropriately, they maintain proper health care delivery to every person. The primary ethical factors include autonomy, beneficence, nonmaleficence, and justice. The code offers guidelines that boost the care continuum and coordination. It allows nurses to practice respect and work with compassion for the dignity of each individual. Also, it identifies the commitment of nurses to the patients. Nevertheless, it provides the necessary skills for nurses to improve their advocacy when interacting with patients (Moore & Frye, 2019). Lastly, it identifies the importance of interdisciplinary partnership hence increasing care coordination among sick people. This analysis indicates how the code of ethics improves care coordination within the healthcare fraternity.

Social Determinant

The success of care coordination requires individuals to identify some of the social determinants which act as vital concepts in the healthcare system. They include the social and environmental circumstances within a particular region. They have been used in determining the well-being of every person in society. Healthy people 2020 has documented social determinants as essential in achieving healthcare programs. Care coordination should use the knowledge of social determinants in addressing the issues related to homeless shelters (Kelley, 2020). The examples of social determinants correlated with care coordination include access to healthcare, resource availability, socio-economic condition, exposure to crime, social support, and availability of community resources.
